We are an enthusiastic and supportive multi-GP training practice with 3 partners: -

Emma Fordham
Simon von Schreiber
Rupert White

We work in modern purpose built premises with air conditioning and lift access to our upper floor. Our current list size is 7350. We have an excellent nursing team comprising 1 Community Matron, 1 Nurse Practitioner, 3 Practice Nurses, Assistant Practitioner and 1 phlebotomist. We also operate minor surgery, substance misuse shared care, and Long Term Condition clinics and an evening weekly Family Planning clinic. We also have Citizens Advice Bureau sessions, a drop in Social Worker session, physiotherapy, diabetic chiropody and Mental Health services within the practice. We house the East Hub for the Extended Access Pilot, offering appointments to 40% of Chesterfield’s GP registered population, up to 8 pm weekday evenings, Saturday and Sunday Mornings.

Our practice population is fairly traditional with many extended families, ex-miners and a small ethnic minority. There have been a number of recent new housing developments with a subsequent influx of young families. We therefore have a balanced, pleasant and rewarding range of patients to work with. There is a high prevalence of chronic disease. High priority is placed on excellent clinical care and we are particularly proud of our 100% QOF achievement. This we credit to our long established organisation of chronic disease management and use of computerised searches to ensure patient follow up.

Great emphasis is placed on team working, care and support of our staff with annual staff appraisals and regular individual team meetings (Admin, Reception and Nursing teams). We have a weekly GP and Practice Manager meeting, quarterly business meetings and try to meet for coffee whenever possible. Monthly QUEST sessions (protected half day training) are held for all staff affording the opportunity to discuss practice issues, host outside speakers, run in-house teaching and present recent audits.

The practice uses Systmone and makes highly effective use of IT solutions such as INR Star, Intradoc and MJOG messaging services. We embrace the move in the new contract for IT usage and hope to pilot electronic patient communications.
The Practice Manager, Paula Elliott and Assistant Manager, Chelsea Webster, oversee the running of the practice and will be able to direct you for any particular issues you may need to discuss.

Practice Ethos:

We are a well-qualified, forward thinking, multi-skilled team. We work from modern, spacious premises providing friendly, accessible services ensuring continuity of care. We work collaboratively within a wider health community and strive to provide quality care closer to home.

We are a five partner PMS Practice with a list size of 9900. We operate from large modern premises that were built in 2006.

We are located in Clowne – just off Junction 30 on the M1 - and are currently the only Practice in the town. Our Practice area covers Clowne, Barlborough, Creswell, Elmton, Hodthorpe, Shuttlewood, Stanfree and Whitwell. These are largely ex-mining villages.

Clowne is a developing town and is evolving into a commuter belt for the main cities of Chesterfield and Sheffield. Over the next five years a further two thousand houses are to be built here.

We have a large friendly team that strives to provide our patients with the best possible care we can. Our team consists of:

5 GP Partners (1 of whom is a trainer)

2 Salaried GPs

2 Nurse Practitioners

5 Practice Nurses

3 Health Care assistants

2 Community Matrons

The clinical team are supported by a skilled and enthusiastic administration team.

Our building is a shared building and we are fortunate to work alongside the District Nursing Team, Health Visiting Team, School Health, Podiatry, Dental and various other community services. This enables excellent communication between the teams and allows for a better quality of service to our patients.

We are a forward thinking practice that has an excellent reputation within North Derbyshire CCG. We are keen to progress and are often involved in various projects and pilots for the CCG.

We became a training practice in December 2015 and are extremely enthusiastic about this. We have taken on medical students since 2011. Our expectation is that our Registrars become an integral part of our team. We have an open door policy and strive to achieve a relaxed, informal but productive and informative working environment.

We hold weekly clinical meetings, palliative care meetings and monthly training sessions. We hold a daily 11am meeting for the GP team where any current issues can be discussed.

Our partners’ special interests cover Palliative Care, Musculoskeletal Medicine, Substance Misuse, Women’s Health, Minor Surgery, Dermatology and Prescribing Three of the partners undertake committee and governing body roles within the CCG and one partner is on the Board of Directors for North Derbyshire GP Federation.

Our Practice Manager is Janina Gawel and she will be happy to answer any questions you may have about us or our Practice. Contact number: 01246 819444 or email nina.gawel@nhs.net
