Practice Profile:

The practice operates from purpose built (1994) premises in the centre of Matlock with 4 GP Partners and 2 GP Assistants. These house not only the practice staff but our entire district nursing & health visiting, all of whom have direct access to our EMIS Web computer system through which we try to work as paperless as possible. Close by is a strong and accessible Community Mental Health Team. Monthly MDT meetings are held including representatives from local social services.

Monthly Palliative Care meetings are also held with representation from palliative care nurses at the local Hospice.

The practice has teaching status for medical students, GP Registrars, nursing and health visiting students.

Appointments for doctors are booked up to two weeks in advance. A Duty Doctor is booked each day for urgent cases. Appointments for nurses can be booked in advance. We offer late and early appointments on Monday and Tuesdays which can be booked online via patient access. Prescriptions can also be ordered via patient access.

We care for approximately 8000 patients in Matlock and 2000 patients in Ashover and operate all usual clinics one would expect to find.

One mile away lies Whitworth Hospital to which the practice has a significant commitment. There are 12 - 28 well used GP beds on Oker Ward. The doctors perform weekly ward rounds to oversee care of patients on the ward.

The hospital has a minor injuries unit seeing approximately 7,500 new attendances per annum.

Our General practice out of hours work is conducted through Derbyshire Healthcare United which covers the whole of Derbyshire including the Peak District and all of the community hospitals.

Matlock has been a training practice since 2009. GP Registrars have their own consulting room and work closely supported and in addition to GPs.

A medium sized, well-motivated Eckington based practice in modern purpose built premises, serving large local estates and surrounding villages. Situated 7 miles north-east of Chesterfield, 8 miles south-east of Sheffield & 3 miles from the M1 motorway.

Moss Valley Medical Practice is part of The Valleys Medical Partnership and we have a second site at Gosforth Valley Medical Practice in Dronfield, which is also a training practice.

The site at Moss Valley has a team of 8 GPs, all of which are GP trainers or have completed or are planning to complete clinical supervisor training. We have an Advanced Clinical Practitioner, 3 practice nurses and 3 HCAs. We also have a team of 11 administrators, supported by an Assistant Practice Manager.

This site shares the services of a full time Pharmacist, part time Phamacy Technician and a full time Practice Manager with our site in Dronfield.

Moss Valley accommodates a team of up to 4 GP Registrars with a structured timetable including daily debriefs, weekly tutorials, weekly clinical team meetings and protected study time. Weekly tutorials are sometimes conducted along with GP Registrars from our site in Dronfield.

GP Registrars are encouraged to become an integral part of the Practice Team and are expected to take a fair share of the workload and responsibilities.

The Valleys Medical Partnership also accommodates up to 4 medical students (2 at each site), an F2 Doctor (at Moss Valley), a Physician Associate (at Moss Valley) and student nurses (at Moss Valley). We also provide full extended access services on behalf of 5 local practices during the evenings and at the weekend.

We are a five partner PMS practice with 11400 patients and currently employ 5 salaried GPs.

The surgery is located in a mixed residential area situated on the northwest of Chesterfield.  The practice has a diverse patient population with unemployment and non-owner occupier rates higher than in surrounding wards. Although predominately urban, our practice area extends to some rural villages on the edge of the Peak District. The surgery is approximately 15minute’s drive from the M1 and has good road and rail links to Sheffield, Nottingham and Derby.

Our multi-disciplinary team consists of Nurse Prescribers, Practice Nurses, Health Care Assistants and administrative support who compliment the delivery of clinical care dealing efficiently and politely with enquires from patients, health professionals and other agencies.  GP Registrars are encouraged to become an integral part of the team being well supported with daily debriefs and weekly tutorials.

The partners meet informally each day with members of the clinical team and more formally, once a month with the wider team, to discuss and review practice business. Each Partner has responsibility for particular QOF indicators, clinical areas and non-clinical roles such as staffing, finance, training, IT and CCG.

The surgery is open weekdays 8.00am to 6.30pm and provides an extend hours service till 8.00pm on Tuesday and Wednesday evenings. We also provided extended access till 8pm.

each weekday plus Saturday and Sunday mornings via the collaboration Hub.  The surgery operates a 48/24 hour access system with 10 minute consultation slots.  Patients can book on the day with some appointments also available to book up to 4 weeks in advance.  The number of home visits vary each day but typically average one to two per doctor.

The Practice achieves high Quality and Outcome Framework scores, participates in national and locally enhanced services, and does well in patient surveys.

We run a variety of disease management clinics with all clinicians involved in audit, developing protocols, recalls and the implementation of NSFs, NICE Guidelines + PCT protocols.

The practice uses SystmOne clinical software system, with electronic referrals, scanned workflows and lab links. We operate a practice intranet to provide up to date information in a paper light environment.

Chesterfield is well served by a modern district hospital, and relationships with local consultants are excellent. We are members of the Chesterfield GP Collaboration working closely with all Chesterfield Practices.
